Early Alert Form

Care Reports are the new early alert notification system at the College to identify students who are ‘at-risk’ academically (performing below a “C” level or in danger of failing), exhibit a behavioral concern, or demonstrate a concern for his/her well-being. The goal of this college-wide initiative is to increase the retention and success of students at CCC.

There are two components in the Early Alert Process.

 Component One: Faculty Intervention (most effective!)

As faculty you have the opportunity to discuss with students their academic progress in your course(s). If a concern has arisen, please develop a plan of action to help the student to succeed. Reach out to the student, talk with them before or after class, call, email them at their college email.

Please do not refer students with only *academic issues that you have not attempted to discuss with the student! Time is of the essence and the faculty member is the best and most expedient resource to help a struggling student succeed. 

Keep in mind, the earlier that outreach occurs the greater chance for assistance and resolution.

If after at least two (2) unsuccessful attempts to contact the student have been made by the instructor, then proceed to the next component.

 Component Two: Referral for additional Support Services

If the issue/concern persists, faculty can then refer students to counseling. Referrals can be made by completing the Counseling Referral Form below
